Buy a SCORE Application Form for Rp125.000,00 (CIMSA members) and Rp150.000,00 (Non-CIMSA members) to BNI 0790833349 a/n Muhammad Ivan Pratista.

Fill in the form below (Identity and the proof of payment)

Wait for an email containing AF, CV, motivation letter, & terms and conditions SCORE CIMSA Indonesia templates. They are sent to your email that is registered in the form.

Pay attention to the instruction written in the email.

Fill in Application Form and Curriculum Vitae, sign terms and conditions SCORE CIMSA Indonesia, and write a motivation letter correctly before sending them back via email to lorecimsaugm@gmail.com.

  • April 19-28, 2021Open Application Form

    Check and follow the rules and regulations to fill out the application form.

  • April 28, 2021Deadline Submission

    Submit the required documents on this date at the latest.

  • May 10-16, 2021Interview

    Applicants will be interviewed by the LORE in their respective.

  • TBAAnnouncement

    The selected applicants will be published on SCORE website.

RULES AND REGULATIONS

The applicant must be a medical student, at least in the 4th semester and graduated from medical school for less than 6 months when departing for the exchange program.

If the medical student’s university doesn’t have a SCORE local, the student may apply him/herself in the nearest SCORE local (you may check the SCORE referral list to see your nearest SCORE local).

Has read and understood the exchange condition of each country. You can access the exchange condition for each country from here.

The application form is only valid for this exchange period (SCORE Exchange 2022/2023 which is from April 2022 – March 2023) only.
